VTep exists to bring evidence-based and practice-informed training directly to the teams who need it—on site, in their own operating rooms (ORs). RCBDE provides a surgery first single-stage, minimally invasive solution for common bile duct (CBD) stones identified at the time of cholecystectomy. RCBDE training delivers a robust stepwise approach to establish sustainable surgical independence, decrease transfers, and strengthen a hospital’s acute care surgical capability. Our comprehensive program is designed to enable surgical teams to comfortably and immediately integrate RCBDE into daily practice.
This surgeon-led course will define the use case for RCBDE and rationale for a surgery first approach. Over a half-day, didactics will focus on how to execute step-up surgical management of CBD stones in patients with a gallbladder and how to build a complex cholecystectomy and RCBDE program with sustainable referral patterns.
Participants will become well-versed in the following:
- Robotic Intraoperative Cholangiography (IOC): Execution and Interpretation
- Wire-Ready Cholangiography
- Step-up Transcystic CBDE Intervention Algorithms
- Antegrade Fluoroscopic Balloon Interventions
- Choledochoscopy: Single and Skilled Assist Approaches
- Advanced CBDE Maneuvers (i.e., lithotripsy)
Bedside Assist Intervention Training:
This course is run by a lead bedside assist who is an expert in the bedside management of RCBDE interventions. Over several hours, bedside assist participants (eg., CST, FA’s, PA’s, etc) will receive both didactic content and hands-on training for patient positioning, C-arm placement while docked, wire-ready cholangiography, RCBDE interventions including wire troubleshooting and balloon management in addition to choledochoscopy interventions.
OR Suite Simulation Training:
This multi-faceted simulation training will take place in the actual OR. All course participants are included: surgeons, bedside assists, C-arm technicians, and circulating nurses. Training will focus on OR deployment, C-arm integration, and step-up RCBDE intervention simulation on a 3D model with the Da Vinci surgical system.
Participants will simulate all aspects of RCBDE:
- Robotic IOC Execution While Docked: IOC Catheter Placement and C-Arm entry
- Wire Access, Troubleshooting, and Wire Control
- Balloon Sphincteroplasty and Snowplow
- Choledochoscopy and Lithotripsy
OR leaders and support staff will be trained on the mechanics and best practices of implementing an RCBDE program. The goal is stakeholder alignment to alleviate operational chokepoints and set clear expectations.
